  • Outreach support services

Referrals can be made to asphaleia care for outreach services to young people who are already living in the community and need extra support. This includes young people who have lived with asphaleia previously and moved into the community. There are three different levels of outreach available with weekly payment packages available:

Level 1: Input appropriate for a young person who has high needs and also multiple tasks within their Care plan. As well as this it maybe that regular contact is required weekly to maintain and monitor progress closely. Overall weekly contact time with a key worker would between four and six hours per week and also taking into consideration travel time for an outreach worker. For those young people with more intensive, complex needs, the weekly support package can be flexible and designed to ensure their needs are met.

Level 2: Medium level supporting a more independent young person with specific needs which could include appointments, drop in visits or key work. This would involve up to five hours of key work support per fortnight.

Level 3: This low level of support is more tailored for a young person that is very settled with low needs. Telephone contact is more prevalent with occasional home visits or contact made. With telephone contact this would approximately be one hour per week (with occasional contact time planned during the specific time of support).

The outreach worker would visit the young person at their home and can also attend any appointments they need assistance with.
